In connection with the implementation of offline or face-to-face lectures for the even semester of the academic year 2021/2022, hereby submit the attached form of permission to take part in offline lectures to be filled out and signed by the student's parents.

This form must be filled out by active students and submitted at the link https://bit.ly/ijinkuliah_luring no later than January 11, 2022 Students who take courses offline (face to face) must have complete vaccines (dose 2).
